At Appalachian Wellness, LLC, coordinating your primary care needs has become easier than ever. Our personalized primary care scheduling allows for you to have behavioral health or medication-assisted services and attend your primary care appointment on the same day at the very same location. Upon completing your intake assessment, you will be scheduled with our primary care provider, Jade Campbell, APRN or Dr. Alan Sizemore for a comprehensive health screening. In addition, they are prepared to manage your ongoing chronic care and primary health needs.
